The most obvious is income. People with LTCs are more likely to be poor in the first place as poverty is a major determinant of health (Acheson 1998). This is further exacerbated by the fact that they may be forced to give up employment or already be retired, and is compounded by the possibility of the carer also having to relinquish a paid wage, forcing him or her on to state benefits.
The pattern of LTCs in ethnic minority groups may be determined by genetic or cultural factors. There is a higher prevalence of diabetes in the South Asian population, for example, and cultural attitudes towards diet could be responsible for the increased or decreased prevalence of heart disease (Chowdhury 2003). Institutional racism means that people from ethnic minority groups tend to have lower-paid jobs and live in poorer areas while fear of racist attack can lead to social isolation, anxiety and depression.
Depression is thought to affect about half of all people with Parkinson’s disease (National Collaborating Centre for Chronic Conditions (NCC-CC) 2006). Mayor (2007) makes the point that the diagnosis of mild depression can be difficult to make in Parkinson’s disease because of the overlapping of motor symptoms with features of clinical depression. She also notes that depression is the single most important predictor of quality of life in Parkinson’s disease, and can impair efforts to control symptoms and cause tension with carers.
